Renew Permits Online

At this time, only Food Service and Tobacco Establishment permits are able to be renewed online. Please be advised that changes are not able to be saved on the online permitting system until submission of the application.

All other permits must be renewed either in person or by mail using the fillable PDF documents. Below, you will find the fillable PDF documents for various public health permits. Once you have completed the correct form, please refer to the fee schedule to complete the permit application/renewal process.

Medical Spas, Nail Salons, Hair Salons, Barber Shops, etc.

  • Medical Spas, aesthetics, cosmetology, manicuring, electrology, barber shops, and hair salons are not regulated by local boards of health in Massachusetts.
